Who is the Transition Team?

We are a team of educational professionals knowledgeable about providing transition information to high school students who receive special education services.

We work along side the home school professionals who know the students best.

Helping Students Prepare for Success

When students graduate from Ohio high schools, they must be ready for success in college and careers without needing to take remedial classes. This goal is measured by the Prepared for Success component.

What we do and why?

We meet individually with the student and their teacher(s) (and possibly school counselors) to discuss their post secondary goals, aptitudes and areas needing improvement.

The National Secondary Transition Technical Assistance Center (NSTTAC) identified that to promote student self-determination, involving the student in transition planning has potential for positive transition outcomes.
